OBGYN Appointment Scheduling

We welcome the opportunity to serve you and provide you with the best OBGYN doctors available in your area. Routine visits are by appointment only. To make appointments, please call us at 772-261-9636. Please have your insurance information available. Some insurance companies require that you have authorization to see us from your primary care physician, particularly for problem visits. We ask for your assistance in obtaining this authorization from your primary care physician or insurance company.

If you have internet access, please complete your patient information on our Patient Portal prior to your appointment date.  You will need to arrive at least 30 minutes prior to your scheduled appointment time to allow adequate time to fill out your patient information and medical history forms.

Before you arrive

You may find it helpful to write down in advance any questions you may have for the doctor or midwife. Bring a list of medications you are taking, or bring the prescription bottles with you so we can accurately document these for our records.

It is helpful for us to have a copy of your medical records from your previous OBGYN physician, particularly if you have had any significant medical problems or surgery in the past. Please contact that physician’s office to have your records sent to us.
On the day of your visit, please arrive ten to fifteen (10 – 15) minutes early – thirty (30) minutes early for new patient appointments. Please bring your insurance card and if available, your prior medical records.

When you arrive

When you arrive for your appointment, we will ensure that we have all the address and insurance information necessary to process your claim efficiently. A staff member will obtain your blood pressure, weight, and other preliminary information for the physician. She will then escort you to the exam room and prepare you for your visit with the physician or midwife. During your visit with the physician, please feel comfortable to ask whatever questions you may have. Whether you are seeing us for an annual wellness exam or a specific problem, we want ensure that you have all of the information you need and understand your future plan of care. Your input into this process is very important. If you have any tests performed, the physician will let you know when we will report those results to you.

If you find that you cannot keep your appointment, kindly notify us as soon as possible. This courtesy allows us to provide care for another patient.

We look forward to caring for you!
